Teen Volunteer Application 
and Placement Process
Thank you for your interest in volunteering at UCHealth at University of Colorado Hospital .Teen Volunteers play an important and meaningful role in providing amazing service to patients and their loved ones. We hope you find your experience here to be valuable and worthwhile.

You will need to complete the following items before we can schedule and interview.

- Download 2 reference forms to be uploaded or brought in

1. Complete the online application before submitting. Once you have submitted your application you can then Login to your record on the website to access more information and upload 

Additionally, we need a signed parent/guardian consent form and 2 references. Your reference forms may not be completed by a relative. We prefer references from adults, supervisors, teachers, co-workers or someone who has known you for a long time. We must receive these references before you may begin volunteering. 
2. Please upload your references under the Teen Reference Upload option under You may scan and email your references to: sara.bencomo@uchealth.org or jennifer.ricklefs@uchealth.org. You may also FAX to 720-848-1885 or mail it in. Please do not send your background check form. You may bring it with you to your interview. 
3. We will contact you within three business days to schedule a one hour interview. At that time we will discuss your application, placement interests, schedule availability, and any questions you might have.
4. Attend a mandatory 3 hour orientation. These are held two times each month.
5. Complete a Health and Drug Screen (30 minutes) with a UCHealth Occupational Nurse at our Employee Health office. There is no charge to you. (During flu season, you must have a flu shot to volunteer If you need to receive one, Employee Health with give you one with your parent of guardian's permission) 
6. Once we have received the results of your background check, you've completed orientation and a health screen, and we've received two references, your volunteer placement will be made.
7. We request a minimum commitment of  60 hours for the semester plan or 100 hours in one year's time. Most volunteers are scheduled for one day per week for a 4 hour shift.  We will work with your school if your time here can be part of a class project. We will submit hours if you need to volunteer to graduate.
